Like I've stated before, this is primarily fantasy.
Josh looks sixteen, but he's actually over five hundred (maybe six hundred by that point in the story).
Alec is around twenty-to-thirty years old, but looks about sixteen as well. Her being a witch just suggests extra powers.
They are all Riddles—if you've read Death is Unpredictable then you'll realize that they're this kind of mythical being that is part of the afterlife.
