I posted about this a few months ago. Gerry Sandusky is the Baltimore Ravens radio announcer, and he had a lot of trouble when the sex scandal involving Penn State's defensive coordinator Jerry Sandusky came out. Both involved in football, exact same last name, first names sound exactly alike but are just one letter off.

My point is that even with an uncommon last name, there is still a risk.

Having said that, if my last name was Smith, I wouldn't call a child John Smith or Mary Smith. But I also wouldn't deliberately try and choose a name that was especially uncommon for its own sake. I would just use the name I happened to like best anyway as long as it didn't sound funny with the last name or was the name of a famous person. No Seth Smith, and no Patty Smith, for instance.
