I have seen Allexzhander. *Shudder*
And I really cringe when I see names spelled totally against all pronounciation rules:
Chatarina (Katarina)
Cewin (Kevin)
Kacper (Kasper)
Candra (Sandra)

And unnessecary "tinsel" in names, like spelling Anna Annah or Annha.
