I really like it... I used to be fascinated with it, ever since I saw it in a Robin Hood book. It's definitely a real name, with lots of history. With a solidly masculine "real" middle name, I think it would work really well (Guy Roderick, Guy William, Guy Alexander, Guy Magnus, etc.)
