Teachers in their junior school might get a good laugh but I don't think the child or their classmates will "get it" until the child is old enough to have realised for themselves and not really care because it's their name.
I use my initials quite a lot on school/college work because they're really distinguishable (M.M.M) so I suppose you'd have to think about how frequently their initials are going to be used and then think about if it's that big a deal.
