Fifth Child
Siblings have a mix of surname as first names or a Grandpa name with a short, recognizable nickname. Possibilities include Arlo, Walker, Murray, Theodore but none quite fit the bill. Girl name would be Esmé.

I was watching a history show today and looking into someones genealogy today. The fun names of the day I have are Alpheus nn "Alfie" and Theobald nn "Theo".I also saw an Amos, Alphonse nn "Alfie" was something I thought of because of Alpheus. There was a boy named Diadem which was unusual, there was also Whitaker, Milton, Cornel, Solomon, Lonoza, Wallace, Ashbel, Amariah, Benajah, Eliphalet, Helier, Ambrose, and Elfego.I'd try harder to help but I decided instead to give you are group of names that mostly fit your criteria that probably no one would suggest.

It's a lot easier to come up with a name when you post the actual sibling names, just so you know. Style helps but it's hard to get the right name for the vibe.
You could try:
Leopold
Albert
Ernest
Francis
Thorpe
Abraham
Browne
Gardner
TannerAll these names seem to me to fit your "older names/surnames as first names" theme.
