Eliot/Elliott were pet forms of the name Ellis or Elias in medieval England. They did not start out as surnames, to my knowledge, but later became far more popular as surnames than as first names. That said, I'm not sure that I'd view Eliot as a "medieval name," per se, as it's experienced such a renaissance of late.
