Well, I don't know about German, but in Croatian and other Slavic languages it's very, VERY rare. The only example I can think of is the Serbian tennis player Novak Dokovic. Novak is a very common last name, and this is the only time I've seen it as a first name.

In Croatia it only happens if the last name is also a first name, like Marko, Ivan, etc.
