Until recently there have been so few "IV's" that a traditional nickname hasn't developed.
But you could use Quad or Delt, short forms of the Latin or Greek words for "four", as Trey is from the Latin for "three". Or if you want to get more exotic, perhaps Ket, a short form of keturi, the Lithuanian word for "four".
