It can't possibly be the Slovenian form of Blaise because this means "tender, delicate, gentle" from Slavic "blag". There are also names Blaža, Blažo, Blažan, Blažen, Blaženko, Blažija, Blažimir, Blažun, Blaško all of which derive from the word "blag".

Blaž is also the Croatian form. It's cognate Vlaho is used mostly in southern parts of Croatia. [noted -ed]

Blaženka (male Blaženko) and Blažica are Croatian feminine forms of Blaž, however they are usually said to derive from the Slavic word "blag" as explained in one of the previous comments. [noted -ed]
