Both Paul and Anthony are great names that can go anywhere and belong to anybody. Anthony as a first name might be problematic if your stepson is Anton; but Anthony for a middle name shouldn't be any trouble.
If you used Paul as the first name, would you call him Paul or by his middle name? Because really don't get the point of intentionally, from the start, calling a kid by his middle name. My thinking is, the name you are going to call him should be the first name, and later on if he decides to go by his middle name, well, it's kind of out of your hands.
Paul is also a fine middle name if you would rather not call him Paul on a daily basis. I think that using a variant like Paulus would not be honoring him as much as using one of his actual names.I think, therefore I judge.

If we'd use Paul as a first name, we would just call him Paul. I agree with you that calling your child by his or her middle name doesn't make a lot of sense.
