Remembrance is a word-name. It has various senses, but the main meanings are "keepsake, souvenir" or "memorial". The two links below give more specific information on the origins and meaning of the word.

I've never heard of it being used as a name before, but there are quite a lot of once-off word names out there. They were more common in earlier centuries, particularly amongst Puritan families.
