I always think of it as a surname, and specifically the surname of the Physical Science "teacher" at my school, who couldn't teach a puppy to yap but never tried to improve, with sorry results for our final exams. We couldn't understand, still can't, why nobody fired her and found her a job where she could do minimal harm, sweeping the streets springs to mind.
