Cousins and other extended family members can have the same surname. I think the shared middle name thing signifies children of the same immediate family and generation. It's not uncommon in some cultures. Not that I approve of Angel Angel Angel Angel Angel Angel Angel Angel, but the concept shouldn't boggle the mind.
