Head is a real surname. Famous bearers of the surname include actor Anthony Stewart Head (Rupert Giles from Buffy the Vampire Slayer) and his elder brother Murray Head, who is not only an actor but also a singer. One of his best known songs is One Night in Bangkok, which was a hit in 1984.With that said, it is indeed extremely unlikely to encounter a Dick Head in this day and age (even if only because Dick is a very old-fashioned name that you are unlikely to see on millennials and zoomers). However, that does not mean that there has never been at least one Richard "Dick" Head in the past. Dickhead as a pejorative was first attested in the 1960s, so it is perfectly feasible that at least one (more likely several) Richard "Dick" Head existed before the dawn of the 1960s. One of these men might even still be alive! A very distinguished musician, choir master, conductor and broadcaster on classical music radion here in South Africa is Richard Cock. No nickname that I know of, especially not Dick, but I do think his parents were taking a chance. He'd be, probably, in his 60s or 70s by now.
