Piguleńska looks like altered version of Pigulińska, which is female version of Piguliński.
Posibbly inherited from family or merriage.
Piguliński - from Piguła, word "piguła" literally it is a big, hard to smallow pill, it is also a joking, kinda rude way to call a nurse or apparently in past apothecary (chemist), so it's sort of a profession surname.
