as an uncommon/interesting middle name to liven up Jonathan? And what would he go by? Initials? A diminutive
of Jonathan? His middle name, etc?

I would use Jonathan Avery and he'd go by his middle name.

Uh...

I'll go with my first instinct. Jonathan Cymbeline. Called Jonathan. Or he could go by Jay, Jon, Joe, Nathan, JC, Cymbe (Simba), Bela, if he wanted.
